Overview

About

The renin-angiotensin aldosterone system is the therapeutic target of first-line antihypertensive medications angiotensin-converting enzyme inhibitors (ACEi) and the angiotensin receptor blockers (ARBs). Comparison of the effects of these drugs on various clinical parameters will be the focus of the discussions in this course. The two lectures in this module will cover Hypertension in Special Population: DIABETES and How We Can Explain the Differences Between ACEi and ARB: Focus on Pathophysiology.

Learning Objectives:

  1. Explain the influence of hypertension and diabetes on cardiovascular disease, including the importance of tight control of blood pressure in patients with diabetes
  2. Evaluate the evidence for ACEi and ARBs in patients with hypertension and diabetes on cardiovascular morbidity and mortality
  3. Evaluate the evidence of for ACEi and ARBs in preventing chronic kidney disease in patients with diabetes who are at high cardiovascular risk
  4. Explain the difference in pathophysiology of ACEi and ARBs on angiotensin II receptors, bradykinin, and nitric oxide production
  5. Discuss the evidence for ACEis and ARBs on endothelium function, fibrinolysis and cardiac protection
  6. Consider the clinical implications of the difference in basic science between ACEi and ARb

  1. Fact check on hypertension and obesity increasing the risk of vascular complications in diabetes
  2. Fact check whether control of blood pressure prevents cardiovascular morbidity and mortality in patients with diabetes
  3. Fact check whether ACEi reduced all-cause mortality, CV mortality, and major CV events in patients with Type 2 diabetes whereas ARB had no benefits on these outcomes
  4. Fact check whether ARBs are superior to ACEi for the prevention of chronic kidney disease in patients with diabetes who have high CV risk.
